Is there a specific form that I need to have that is a medical certification for employees on FMLA leave due to an illness in their family? 



What is the customary practice, in the public sector, for adjusting anniversary dates as they relate to promotions, evaluations and step increases when an employee takes leave without pay for one month or more? 



Can a company mandate the use of paid sick time during an fmla absence when the state provides a mandated disability program? 



Do we have legal obligation to keep paying for health insurance while the employee is not even working for us after two years of workers comp? 



I cannot find anywhere in the FMLA where elective cosmetic surgery falls under FMLA. If I am wrong, is there a website in which I may go to read? 



If a female employee is off on maternity leave using temporary disability benefits from her pension plan must that time count towards FMLA also? 



If an employee is continually leaving early due to issues relating to pregnancy and has not provided medical documentation indicating she should be on a reduced schedule is this time considered FMLA? 



I have an employee in California who has requested a leave of absence for 1 month to take care of her incapacitated mother. Does she qualify for FMLA leave? Does she qualify under the California Family Medical Rights Act? 



Can you give us the formula to use for determining whether an employee has worked a sufficient number of hours over the past 12 months to qualify for FMLA? 



We granted an employee FMLA to care for his daughter. We told him to compleate the forms and return them within 10 days. We heard nothing until he came back 2 mos later with NO paperwork. Were we justified in firing him?
